Of course Tim Henman was once disqualified at Wimbledon for smashing a ball in anger which hit a ball girl.
Greg Rusedski also smashed one up a ball girl and ended up marrying her.
He wasn't disqualified

No he didn't you prat. That came at him at full speed and caught him clean in the eye, which is now badly swollen. Could have caused a nasty injury.

The 18 year old surfer dude Canadian was gonna lose anyway, but losing his rag and smashing the ball in the direction of the umpire was mental. He could never duplicate that shot how ever good he gets.
